Requirements for Employment

  1. Proof of identity including but not limited to Drivers License, State Identification card, US/Foreign Passport, Permanent Resident card, Work  Authorization card or any other Government-Issued ID
  2. Valid Social Security card 
  3. Physical & PPD test (must be completed within six months of application to Premier)
  4. Chest X-ray (if applicant has a positive PPD test history) – must be accompanied by a communicable disease screening form
  5. CPR (Card must indicate Adult CPR. Adult and Child acceptable but not infant CPR as a standalone) 
  6. First Aid
  7. Home Health Aide Certification & Skills Check list from an accredited school in the District of Columbia 
  8. Medicaid Provider Number (for Home Health Aides only) 
  9. For HHAS – current HHA license (license must not have any restrictions) 
  10. For RNs and LPNs – current RN or LPN license (license must not have any restrictions) 
  11. High School Diploma or equivalent or higher (for DSPs only) 
  12. Global Background Check results. If candidate does not have the Global background check results, Premier can initiate the process at a cost of $20.00 cash for every state the candidate has lived and worked in for the past 7 years. All police clearances must be within two months of application to Premier.
