Through its subsidiaries, PHG’s goal is to create a continuum of care from pediatric home health services to hospice services, utilizing an efficient model across services. PHG seeks to provide quality healthcare through a coordinated plan of nursing, ancillary and rehabilitative care. While prioritizing providing each individual and family with education and advocacy opportunities to maximize the service needs of each patient.
